Log in to TxRecord Portal
Goto Menu > Management > Assessment Form
To add a new assessment form, enter the form name, Applications to which the form belongs to, description and pass marks and click on add.
To Add categories and questions to the form, select the form and click on View.
Initially the form name will appear, to add a category, right click on the Form name.
To add a category, provide Name, Description, Weight and Score. Here Score is score per question falling under that category. Weight will be the total weight of that category, this value will be multiplied to total score of that category.
To delete the category, modify the values , right click on the category name.
To add a question to this category, right click on the category name and click on Add Question.
Questions can be of 3 types: Rating, Polar, Option
Type: Rating
Questions of this type will look like below.
Type: Polar
Questions of this type will have Yes, No, N/A as answers.
Yes will be scored full points, No will be scored 0 points , if N/A is selected the assessment form will not consider that question.
Type: Option
Questions of this type can have customized answers. These types of questions will not be scored.
buttons can be used to add and delete options.
Questions of type Option are multiselect and they will look like below.
Once the categories and questions are added, the View of that form will look like below.
All the questions, categories can be modified.
Once the form is completed you can release the form. Once the form is released, it cannot be modified.
